Check-In Forms

These forms are for you to download, fill out, print (in COLOR ink!) and bring with you to your State Pageant Check-In this summer.

Get ready to strut your stuff, finalists! It's time to shine at the state pageant. Don't forget, all eyes will be on you, so make sure you're armed with your completed forms when you arrive for pageant weekend. Whether you're jotting down your details by hand or prefer the sleekness of typed pages, ensure you're equipped with your resume, emcee card, and any other necessary documents.

Hold onto those forms tight; there's no need to send them through the mail. Instead, bring them along with you to pageant check-in, where you'll hand them over in person.

These forms aren't just pieces of paper; they're your ticket to the spotlight! Our emcees will be flipping through them to introduce you on stage, so make sure they're filled with your best achievements, from community service to academic excellence and beyond.

Remember, you're in control here. Only fill out the forms you need. The essentials for the overall pageant include your resume, community service details, and that all-important emcee card. If you're diving into the optional contests, don't forget to download and completed those forms accordingly.

IMPORTANT: For all divisions, lease also complete your emergency contact form!

If you haven't already, please click here to fill out your emergency contact form.

This is required for all contestants to have on file.
