​These forms are for you to download, fill out, print, and bring with you to your State Pageant Check-In this summer. These are PRE-TEEN division forms. Please verify your contestant is 10-12 years old based on her age on January 1st, 2024.

Please ensure all your forms and cards are completed, printed, and ready to be turned in at pageant check-in, as we will not have printers available on-site. While printing in color is preferred, black & white copies are also acceptable. Don't forget to hand in your completed cards when you arrive at pageant check-in. Thank you!
